Pre-Test Requirements: For accurate metabolic testing results, I understand that I must fast for 12 hours before my appointment (water is permitted). On the morning of my test, I will weigh myself immediately after waking and using the bathroom, and provide this weight to the clinician for equipment calibration.
Phlebotomy & Blood Collection Consent: I understand that phlebotomy services involve the collection of blood samples through venipuncture by trained and insured professionals. I acknowledge that this procedure carries minimal risks including but not limited to: minor bruising, discomfort, fainting, infection at the puncture site, or rarely, nerve damage. I confirm that Theorem Metabolic's phlebotomists are fully qualified and insured professionals.
Metabolic Testing & Health Assessment: I understand that metabolic testing (including VO2 Max, RMR, mBCA, and CGM services) provides data-driven insights into my individual metabolism and health markers. These tests are designed to optimise my health and performance but are not a substitute for comprehensive medical diagnosis or treatment by a qualified physician.
Medical History & Conditions: I confirm that I have disclosed all relevant medical conditions, medications, allergies, and health concerns that may affect testing procedures or results interpretation. I understand that withholding this information may compromise both my safety and the accuracy of my personalised health recommendations.
Data Protection & Privacy (UK GDPR Compliance): I consent to the collection, processing, and storage of my personal health data in accordance with UK GDPR and data protection laws. My data will be used solely for providing metabolic health services, maintaining health records, and delivering personalised coaching recommendations. We retain your personal data for 7 years for health service records and 3 years for marketing data. You have the right to access, correct, delete, or restrict processing of your data.
